What is the RPHS Program?

The Rural Primary Health Services (RPHS) Program aims to improve access to a range of primary and allied health care services and activities for rural and remote communities.

The program gives community-based primary health care organisations greater flexibility in the range of services they can offer, including health promotion and preventative health activities. 

Who fits the eligibility criteria?

Those who

• require the development of a multi-disciplinary care plan with a focus on prevention of long-term complications and/or;
• may not necessarily have the opportunity to access the services of an Allied Health Professional and;
• live in either Yeppoon, Emu Park, Gladstone or Boyne Island and;
• have a current diagnosis of a chronic disease (of greater than 6 months) and;
• hold a concession card (health, pension, student)

What is the referral process?

General Practitioners can use an RPHS Referral Form to refer an eligible client to the service.

The sessions are provided by appropriately qualified Allied Health Professionals including:

• Diabetic Educators
• Dieticians
• Exercise Physiologist
• Physiotherapists
• Podiatrists

Allied Health Practitioners will provide the referring GP with a summary of the client’s progress within 2 weeks from the date of the final session.
