The Closing the Gap (CTG) program was developed to assist with supporting better health outcomes for the Aboriginal and Torres Strait Islander population of Australia.

There are four (4) key messages that the Capricornia Division of General Practices’ CTG team is trying to reach our Aboriginal & Torres Strait Islander peoples in order to assist with long term better health outcomes for our local communities. Some of the key messages include:

  • The importance of Self Identification for Aboriginal & Torres Strait Island peoples
  • 715 Health checks for ALL Aboriginal & Torres Strait Island peoples
  • CTG Scripts – Prescriptions under CTG program
  • Allied Health Service entitlements (following a health check)

The CTG team continues to work with local community based organisations, local businesses and individuals to promote the benefits and importance of having a health check and seeing a medical practitioner regularly.

The Capricornia Division of General Practice’s CTG team provides ongoing community support, promotion and education to assist with providing greater long term health outcomes and close the life expectancy gap within a generation.
